A code always belongs to one specific platform, so the first thing to check is where the person actually plays.

- PlayStation Store — games and add-ons for PS4 and PS5, plus PlayStation Plus membership.

The code region has to match the account
This is the single most common reason a code fails to redeem: the code must come from the same region as the account. A code bought in Poland works on an account set to the PL region with PLN as its currency, and gets rejected by a German one. The same rule applies to the PlayStation Store and the Nintendo eShop.
